ALL prospective retail customers must complete the Registration Form, even if you plan to pay with a credit card. If you intend to pay with a credit/debit card, type N/A in the fields that ask for your bank information and credit references.

We MUST have your federal tax identification number. Once we receive your registration form, we will email you access to our online catalog. We ask that you place your first order online. Initial wholesale order requirement is $500. Our delivery turnaround time is approximately two weeks.
